Welcome to the PennVet Admissions Supplmental Information Form
This PennVet Admissions Supplemental Information Form is for the School of Veterinary Medicine applicants to provide additional information to supplement the VMCAS application. This Supplemental Information Form and payment of the $75.00 processing fee must be completed by September 15th, 2016 at 11:59PM E.D.T. for your application to be complete. **Please note that you will not be able re-enter the Supplemental Application Form after submission. You may not save the form partially complete. If you navigate away from the form before completion, you will need to begin the form from the beginning. Once submitted, you may not re-enter your form to edit. Because of this, please do not begin the form until you can dedicate 10 minutes to complete it. Do not re-submit a second form, as it will result in duplicate records in our database. If you intend to pay your application processing fee by credit card using Visa, Mastercard, Discover or American Express, you must do so
after submission of the Supplemental Information Form. If you wish to pay by check, you may submit the Supplemental Application Form and send a check as instructed when the Form is completed.
